RevIVe U – Ottawa Mobile IV Therapy

Welcome to RevIVe U. By booking or receiving services from RevIVe U, you acknowledge and agree to the following Terms and Conditions. Please read this agreement carefully before proceeding.

1. Our Services

RevIVe U offers mobile intravenous (IV) therapy services, including hydration, vitamin infusions, and wellness support, administered by licensed healthcare professionals in the comfort of your home, office, or preferred location within our service area.

Our services are designed to promote wellness but are not intended to diagnose, treat, or cure any disease. We are not a replacement for medical advice, diagnosis, or emergency care.

2. Eligibility

To receive treatment from RevIVe U, clients must:

• Be 18 years of age or older, or provide parental/legal guardian consent if under 18,

• Complete a health screening and consent form,

• Not be pregnant or breastfeeding (unless approved by your physician),

• Not be experiencing a medical emergency or acute illness requiring hospitalization.

We reserve the right to refuse treatment if it is deemed medically unsafe or inappropriate.

3. Appointments & Cancellations

• Appointments can be made online, via phone, or through email.

• 24-hour notice is required for cancellations or rescheduling.

• Cancellations within 24 hours or no-shows may be charged $50 or up to 100% of the service cost, at our discretion.

• We may reschedule appointments due to unforeseen events, such as illness, emergencies, or extreme weather conditions.

4. Payment Policy

• Payment is required at the time of booking or service delivery.

• We accept credit cards, debit, e-transfers, and other approved payment methods.

• Prices are listed in Canadian dollars (CAD) and subject to applicable HST.

• Services provided by RevIVe U are not covered by OHIP. Check with your private insurer or HSA provider for possible reimbursement.

5. Informed Consent & Medical Disclosure

All clients must:

• Complete a health intake form prior to treatment,

• Disclose any medical conditions, allergies, or medications,

• Sign an Informed Consent Form acknowledging the potential risks and benefits of IV therapy.

Failure to disclose accurate medical information may result in treatment refusal or adverse reactions. RevIVe U is not liable for issues arising from incomplete or inaccurate disclosures.

6. Risks and Limitation of Liability

IV therapy involves minor risks, including:

• Discomfort, bruising, or swelling at the injection site,

• Inflammation, phlebitis, or infection,

• Allergic reactions to ingredients.

To the extent permitted by law, RevIVe U and its staff are not liable for any adverse effects unless caused by willful negligence. All services are voluntary and undertaken at your own risk.

7. Privacy and Confidentiality

We respect your privacy and protect your personal and health information in accordance with the Personal Health Information Protection Act (PHIPA). Your information will only be accessed by authorized personnel and will never be shared without your explicit consent, except as required by law.

8. Service Area

RevIVe U proudly serves Ottawa and select surrounding areas. Service availability may vary based on your location, appointment type, and staff availability.

9. Modifications to Terms

We may update these Terms and Conditions from time to time without prior notice. The most recent version will always be available on our website. Continued use of our services indicates acceptance of the updated terms.
